Registration for New Students

All students entering EC, K-4 through 4th Grade are required to fill out registration forms (K-4 & new to district K-5 need a physical examination) available at the Elementary School Office at 503 E. Chestnut St., Pardeeville, WI 53954 or at this website. Please call Mrs. Rita Bortz (429-2151, ext. 110) to arrange for a time to register your child.  Please bring the following information with you:

* Registration Form

* Screening Form

* Health History Form

* Birth Certificate - bring an original birth certificate for verification.

* Immunization Record

* K4 or K5 only: Physical Exam Form

* Proof of Residence - provide a copy of a bill or correspondence with your name & address on it.

* Photo Id of Parent/Legal Guardian
